What is double jaw surgery (orthognathic surgery)? Process and recovery

When the upper and lower jaws do not fit each other — or the facial skeleton — properly, the effect is not only aesthetic; it can also affect chewing, speech and breathing. In advanced misalignments that stem from the position of the jawbone itself, where braces (orthodontics) alone are not enough, orthognathic surgery — commonly known as double jaw surgery — comes into play.

This treatment involves surgically moving the jawbones into the correct position, and it is usually planned as a team alongside orthodontics. The goal is a bite in which the teeth and jaws meet in a way that is both functional and balanced.

When is orthognathic surgery needed?

  • A lower jaw that sits noticeably too far back or forward
  • Open bite (front teeth that do not touch when the mouth closes)
  • Severe crowding and bite disorders
  • Chewing and speech difficulties caused by jaw misalignment
  • In some cases, narrow jaws associated with breathing problems such as sleep apnoea

Most of these misalignments become clear once growth is complete. The treatment decision is made through a clinical examination, cephalometric X-rays, 3D imaging and a joint assessment by the orthodontic-surgical team.

How is treatment planned?

  • Orthodontics moves the teeth into position before surgery (pre-surgical preparation)
  • 3D planning determines the new position of the jawbones
  • The operation is performed under general anaesthesia, from inside the mouth (no facial incisions)
  • The jawbones are fixed in the correct position with plates and screws
  • After healing, orthodontics completes the fine-tuning

What is recovery like?

Swelling and mild discomfort in the first few days after surgery are expected and settle over time. A soft/liquid diet is recommended early on, and following the dentist's hygiene and diet instructions makes recovery easier. Solid bone healing takes a few weeks, while full recovery varies from person to person.

Orthognathic surgery is an advanced procedure that requires experience and teamwork. At our clinic these treatments are planned by oral and maxillofacial surgeons in a fully equipped operating theatre.

Not every jaw misalignment requires surgery; mild cases can be corrected with orthodontics alone. The right treatment can only be determined after a detailed examination and planning — consult your maxillofacial surgeon.
